Appointment Summaries/Documentation

  • Who:
    • Advisors complete appointment summaries for each advisee
  • What:
    • Courses you advised for
    • Offices or people you referred the student to
    • Pertinent information that the student must know
      • Scholarship Requirements
      • Probation and Suspension information
      • SAP Appeal
      • Program Admission
      • Account Holds
    • Next steps or instructions
      • Ordering Transcripts
      • Completing Change of Major
    • Advisors can add more information as they see fit. However, advisors should refrain from including personal and sensitive information in the summary. Advisors should only state the facts and should not evaluate.
  • When:
    • After an advisement appointment
  • Where:
    • Navigate
  • Why:
    • Provides a reference for students to look back at
    • Informs Academic Coaches about advisor conversations
    • Helps other advisors a student meets with (e.g. if the student changes their major)
    • Allows Veteran Benefits to be certified
    • Presents the Provost with information to decide on a student’s request for a withdrawal due to extenuating circumstances.
    • Helps other University offices
    • Documents information for the protection of the Advisor and University
    • Supports assessment data
